Have you acquired your funding for this event, allocation, solicitation?
If allocation or solicitation have the proper forms been submitted?
Will alcohol be served at this event?
If Yes, have you talked with Security at least two weeks prior to event to arrange for security?
If on campus and alcohol will be served, email address of advisor or other staff member attending event is required
This form must be complete and submitted to the Assistant Director for Student Involvement for approval
AT LEAST 2 WEEKS, prior to scheduled event date or event will not be approved and may have to be cancelled!
